The first step to becoming a patient of Wayland Family & Sports Medicine, P.C. is to transfer your medical records from your previous health-care provider to us. You will need to print out our Records Release Form. Fill this out for each provider you have had in the past and mail it to them. They then will send us your medical records. Another thing you need to do is to register your insurance information with us and authorize payment by your insurance company. This form is in two parts, Page One and Page Two.
